This Application Form has been prepared by Cansu Susam , in her capacity as data controller, to ensure that applications made by relevant data owners in accordance with Articles 11 and 13 of the Law No. 6698 on the Protection of Personal Data (“KVKK”) are evaluated and finalized promptly, effectively and comprehensively.
APPLICATION PROCEDURE
In accordance with Articles 11 and 13 of the KVKK (Law on Protection of Personal Data), you can submit your requests regarding the KVKK to our Company, which holds the title of data controller, through the following methods:

IDENTITY VERIFICATION
In order to ensure the security of your personal data, you may be contacted and additional information/documents may be requested within seven (7) days at the latest after your application is received by Cansu Susam, in order to confirm that you are the data owner.
If the requested information and documents are incomplete, the thirty (30) day period specified in Article 13/2 of the KVKK will be suspended until the deficiencies are remedied.
Information and documents submitted for identity verification purposes will be destroyed after verification is complete.

This application form has been prepared for the purpose of exercising the rights stipulated in Article 11 of Law No. 6698 on the Protection of Personal Data (KVKK). Applications are evaluated within the scope of Articles 11 and 13 of the KVKK and the Communiqué on the Procedures and Principles for Applications to the Data Controller, and are finalized within a maximum of 30 (thirty) days.
